The Cardinals would probably be wise to trade him. I really can't see them picking up his option for next year; $15mm is a whole lot of money for a 37 year old starting pitcher with some injury history. It's a weak division that I'm sure they still feel they can win this year, but they need to get younger, cheaper talent in the fold sooner rather than later especially if they feel this is their last season with Pujols.
