In the case of the kick return that went for 6 against AZ it was mental error.
The BigTen Network broke this down on film and clearly showed one of the Iowa coverage unit leaving his lane and going toward where the ball was caught instead of staying home like he should have. The vacated spot was exactly where the returner ended up taking the ball through as he swept to the other side of the field.

Well, they said that the safety (?) who did that wasn't necessarily wrong in cutting inside, but it was that the Hawks played it as if it would be a 'right return'. Which was fine until the return man decided to go left instead.
But you're right in that, had everyone stayed in their lanes, it would be unlikely he would have taken that back all the way.
As for the ST bug, I take a tiny bit of consolation in the fact that it isn't just Iowa. I think most of the "elite" teams have suffered special teams miscues and mistakes.
Last year,
we scored two TDs off of blocked punts and kickoff returns, against what were then considered the top two teams in the Big Ten. This year, Ohio State has been downright atrocious on special teams, Wisconsin has been weak, and Iowa's miscues finally caught up to them in the worst way. Minnesota gave up a KO return, I think Michigan had a punt blocked, Michigan State's awesome fake field goal was setup in part due to lack of faith in the kicker... Hell, we haven't even tried a field goal yet, but we've already had an extra point blocked!
