Maybe I'm wrong, but if a QB's helmet is ripped off by a defender, wouldn't that be an obvious indicator of a facemask or illegal contact to the QB's head penalty?

I'm not a Northwestern (or Pat Fitzgerald) fan, but I probably would have had the same reaction that Fitzgerald did after that no-call by the officials.
 
Not necessarily. While it did look like the Michigan player had one of his fingers hooked onto the facemask, there are instances where the defender's arm could have pulled the helmet off (especially the way it happened last night). Don't think there can be a call on a hit to the head when the QB tries to duck the tackle either.
